Our clinic is by appointment only.
We specialize in refractive surgery such as LASIK. We do not accept appointments for patients who are not considering refractive surgery.
To make an appointment, please fill out and submit the application form below. Also, please read the following instructions before making an appointment.
Notes
- The information you provide on this application form will only be used to contact you to make a reservation.
- This application form can be used to apply up to two days prior to the desired date. (Reservations can be made up to 3 months in advance.)
- Your appointment will be confirmed when you receive an email confirmation from our staff. Please note that your appointment date is not confirmed at the time you submit this form.
- Please note that your preferred date may not be available.
Notes for the examination
- The examination and consultation will take approximately two and a half hours.
- We will use eye drops to dilate your pupils to check the back of the eye. You will be sensitive to bright light or difficulty seeing for about three to four hours after the examination.
- Appointoments cannot be made on our closed days (Sundays, Tuesdays, and holidays) and on surgery days.
